Snoring and sleep apnea are closely associated because obstructive sleep apnea (OSA) may cause snoring. As a busy ENT specialist in Singapore, I see many patients suffering from obstructive sleep apnea. It’s often the sleep partners who bring in the patient! Their partners are fed up of their snoring and have to sleep in separate rooms.

So is there a Connection between Snoring and Sleep Apnea?

Good question! Snoring is basically noisy breathing due to airway obstruction. If your airway is blocked, you’re probably going to snore!

Obstructive sleep apnea (OSA) is a serious condition where you suffer difficulty breathing when asleep. Why does this happen? In OSA, your airways are collapsing and blocking off. Snoring is a very common sign that there is some underlying airway obstruction and likely sleep apnea ongoing.

How do we Diagnose Sleep Apnea?

We use an overnight sleep study to diagnose Sleep Apnea. This is usually done at home with a special monitor on your arm. It measures your heart rate, breathing pattern and body position. In addition, a sleep study analyses your stages of sleep. A sleep study will give information about how severe the sleep apnea is. So this will help your ENT specialist in Singapore decide what is the best form of sleep apnea treatment for you. We now know how important sleep health is to maintain our overall physical and mental health, so it is essential to diagnose and treat sleep apnea early.

The upper airway becomes blocked during sleep when muscle tone relaxes to close off the throat.

The worst position for a snorer is when they lie down flat on their back. This causes the floppy soft tissue of their tongue and palate to fall backward to close off their airway, cutting off the oxygen intake during breathing. Some patients may stop breathing and their partners report prolonged quiet pauses in their snoring. This isn’t a good sign by the way! They are suffering episodes of apnea when their abnormal breathing stops and starts. After a while, the loud snoring resumes when they begin to breathe once more.

Is Snoring and Sleep Apnea Dangerous? Could Sleep Apnea Kill Me?

Snoring itself is not “dangerous” but sleep apnea is likely to worsen with age and weight gain. Obstructive sleep apnea carries a higher risk of heart attacks, strokes, poor control of blood sugar levels and irregular heart rhythms. Other most subtle sequelae include memory loss, earlier dementia development and cognitive impairment.

Patients who already suffer other health conditions such as diabetes, asthma and acid reflux, may find it more difficult to manage their conditions if they are also suffering with sleep apnea. For example, diabetics may need to take higher doses of their diabetic medications if they also suffer sleep apnea.

Although sleep apnea may not kill you directly, it is a risk factor to exacerbate and cause so many health disorders that each of these in turn may indirectly affect your general health and lifespan!

But How Do You Stop Snoring?

Another good question which isn’t so straightforward to answer actually.

There are many risk factors for snoring and obstructive sleep apnea.

Lifestyle factors of snoring and sleep apnea:

Weight gain

Smoking

Alcohol consumption

Physical factors of snoring and sleep apnea:

Deviated nasal septum

Nasal allergies (allergic rhinitis)

Enlarged tonsils

Long uvula and floppy soft palate

Large floppy tongue

Nasal polyps

Vocal cord growths

This shows where your inferior turbinates are located. They are the “radiators” of the nose as they warm and humidify the air that you breathe in. The problem arises when these “sausage”-shaped stuctures swell up too much to block your nose.

So depending on what the snoring or sleep apnea is due to, the treatment will be tailored to suit you.

For lifestyle factors, it would make sense to lose weight, stop smoking and cut down on drinking alcohol.

For physical factors, treatment consists of either surgery or using a CPAP machine. CPAP stands for Continuous Positive Airway Pressure, applied via a nasal or face mask to wear at night when asleep to prevent airway collapse (see picture below).

CPAP wearer using a nasal mask to apply positive pressure to prevent airway collapse.

What about Oral Appliances?

If your snoring and sleep apnea are caused by your jaw or tongue falling backward to close your airway off, then a special mandibular advancement device may be helpful. A dentist trained in sleep assessment would usually be the best person to take care of this for you. A specially-fitted mouthguard would have to be measured and made according to your specifications.

An example of a mandibular advancement device for sleep apnea patients to wear when asleep. This pushes the lower jaw forward to prevent airway collapse.

What about the role of Surgery to reduce Sleep Apnea risk?

Surgery can play a helpful role in the treatment of snoring and sleep apnea for some patients. At our busy ENT clinic in Singapore, we see many such patients but not all of them are good candidates for surgery.

I would say that the following groups are the ones who would benefit more from surgery:

Those with other ENT problems (CPAP’s not going to help here) like:

Recurrent sore throat infections

Blocked nose caused by deviated nasal septum

Blocked nose caused by swollen “sausage”-shaped tissue due to nasal allergies

Those who cannot tolerate CPAP:

Those ideally with a Body Mass Index of 30 or less.

Those whose sleep study did not show severe sleep apnea.

What Kind of Surgery May Help Snoring?

Surgery for snoring may be divided into different levels: Nasal, palate and tongue base.

i) Nose surgery

Nasal surgery aims to improve your breathing and blocked nose in the form of septoplasty and turbinate reduction. This corrects a deviated nasal septum and enlarged soft tissue which narrow your nasal passages. Any nasal allergies should also be treated and this may mean considering a course of sublingual immunotherapy to treat dust mite allergy.

How to improve your blocked nose and difficulty breathing. Type image caption (optional)

ii) Throat surgery

Throat surgery usually takes the form of tonsillectomy with UPPP. The UPPP surgery aims to trim and refashion your crowded throat to breathe better. For many snorers, the main source of the snoring noise comes from their long uvula, that wormlike structure dangling at the back of their throat.

The UPPP surgery aims to open up your overcrowded throat to help you breathe and sleep better.Type image caption (optional)

iii) Tongue base surgery

Sometimes, the tongue is large and collapses backward to block off the airway during sleep. So if indicated, surgery to reduce the bulky tongue base may be performed.

All sorts of surgical techniques have been tried to tackle this issue, the most challenging level of airway obstruction in snorers but have variable success rates.

Some specialised dentists may perform jaw surgery to push the jaw forward to open up the airway at the tongue base level.

The most recent advance in surgery includes implantation of a nerve stimulator to activate the tongue muscles to move forward during sleep, preventing the tongue from falling backward. The electrical current from the nerve stimulator device stimulates the hypoglossal nerve, which controls the tongue muscles. Basically, this acts as a “pacemaker” for the tongue.

Share this blog via:

Have you ever wondered if you or your partner’s snoring is something to worry about?
